Some insurance companies require a designated laboratory, radiology facility, hospital or consultant to be used for their members. As a courtesy to you, we try to verify the facility or the consultant and their affiliation with your insurance carrier prior to obtaining any requested services. If inadvertently a different laboratory, radiology facility, hospital or consultant is used, you will be responsible for the charges for that service.
